Topical corticosteroids have anti-inflammatory, antiarrhythmic and vasoconstrictor properties. Corticosteroids act by stimulating phospholipase A-2 inhibitory proteins—lipectins. These proteins regulate the action of potent inflammatory substances such as prostaglandins and leukotrienes by inhibiting the release of arachidonic acid. Arachidanic acid is produced from the phospholipids of the cell membrane by the action of the enzyme phospholipase A-2. Arachidonic acid is produced from the phospholipids of the cell membrane by the action of the enzyme phospholipase A-2.

Drug Indications

Topical hydrocortisone butyrate is used to relieve the inflammation and pruritus of corticosteroid-responsive dermatoses in adults. It is used in the treatment of mild to moderate atopic dermatitis in pediatric patients 3 months to 18 years of age.


Dosage Administration

For corticosteroid-sensitive dermatosis in adults, apply the cream thinly to the affected area two to three times daily, depending on the severity of the disease. For atopic dermatitis in children 3 months to 18 years of age, apply a thin layer of cream evenly to the affected area twice daily. Avoid applying hydrocortisone butyrate to the diaper area unless directed by a physician. Once the disease is controlled, the cream application should be stopped. If there is no improvement within two weeks of medication, the diagnosis may need to be re-evaluated. In cases where the use of the cream is increased from 2 weeks to 4 weeks, prevention of HPA axis and local side effects should be taken into account. The efficacy and safety of hydrocortisone butyrate use beyond 4 weeks is not established. Do not use hydrocortisone butyrate in occlusive dressings (tight wear) without doctor's advice.

Use in children: Safety and efficacy of the drug in use in children under three months of age have not been well established.

Use in the elderly: Safety and efficacy of the drug in people over 65 years of age have not been well established.


Contraindication

Hydrocortisone butyrate should not be used if there is a hypersensitivity reaction to any of its ingredients.


Side Effect

In the case of first use, irritation, itching, dryness, redness may occur in the applied area of the skin. These effects disappear within a few days as the body adapts.


Precaution Warning

Topical corticosteroids can temporarily suppress the hypothalamic pituitary-adrenal axis when absorbed into the systemic circulation, and some patients may develop symptoms of Cushing's syndrome, hyperglycemia, and glycosuria. The use of more potent corticosteroids, application to large areas of skin, long-term use, tight clothing may increase absorption into the systemic circulation. In children, topical corticosteroids are more easily absorbed. Risk of systemic toxicity is high. If local irritation occurs due to drug application, drug application should be stopped and appropriate drug should be selected. Appropriate antifungal or antibacterial agents should be used if skin infection is present. If the expected response is not achieved, corticosteroids should be discontinued until the infection is completely controlled.

Pregnancy & Lactation

Pregnancy: There are no adequate and well-controlled studies of the use of this cream during pregnancy. Therefore, this cream can be used if the benefit rate of the pregnant mother is higher than the risk of burning. Corticosteroids have shown teratogenicity in laboratory animals when administered into the systemic circulation at low doses. Some corticosteroids have shown teratogenicity by cutaneous application in laboratory animals.

Lactation: No information is available on the absorption of corticosteroids when applied to the skin and their presence in breast milk at detectable levels. When applied in systemic circulation, it is present in breast milk at levels that produce no harmful effects on the infant. However, caution should be exercised in the application of corticosteroids to the skin during lactation.
